Substance Use Prevention and Public Health Part 2: Building a Public Health Approach to Prevention Josh Esrick, MPP, and Emily Patton, MSc, PgDip September 7, 2023, 1:00pm-2:30pm EST COURSE DESCRIPTION This webinar will discuss how substance use prevention professionals can build a public health approach to prevention. It will describe how the similarities across public heath fields can lead to specific collaborative opportunities and will identify examples of these opportunities. The webinar will also review the concept of integrating behavioral and physical health care services and how, among other benefits, integration can support the development of a comprehensive public health approach to prevention. Additionally, it will discuss strategies for prevention professionals to further develop this approach and how to reduce health disparities through it. The webinar will also discuss the barriers and challenges prevention professionals may face and the opportunities that may exist to overcome them. Lastly, the webinar will offer a small group peer learning discussion opportunity. LEARNING OBJECTIVES Identify opportunities for collaboration across public health service fields. Mr. Esrick has over ten years of experience researching, writing, evaluating, and presenting on substance use prevention and other behavioral health topics. He is an expert in providing training and technical assistance (TTA) in substance use, having overseen the development of hundreds of TTA products for numerous clients, including six of SAMHSA’s ten regional Prevention Technology Transfer Centers (PTTCs), the PTTC Network Coordinating Office, the Central East Addiction Technology Transfer Center (ATTC) and Mental Health Technology Transfer Centers (MHTTC), and SAMHSA’s Center for the Application of Prevention Technologies (CAPT). These trainings and products have covered a wide range of topics, including strategic planning, data collection and analysis, and identifying evidence-based prevention interventions for youth. In addition to TTA, Mr. Esrick has directly provided many of these services to behavioral health agencies and other entities. He has published several academic journal articles. It will describe the idea of public health and the similarities that exist across its service fields, such as: a focus on preventing issues before individuals require clinical services, seeking to address environmental factors and the social determinants of health, reducing health disparities, and implementing population-level services, among others. The webinar will also explain how substance use prevention should be and is a critical part of public health. As part of this, it will describe the best practices from public health that prevention professionals can incorporate into their work. Lastly, it will define what a “culture of prevention” is and how prevention professionals can take lessons learned from other public health fields to expand it. COURSE DESCRIPTION This training analyzes the co-occurring epidemic of substance misuse and suicide and discusses how to navigate and overcome the barriers that have impacted professionals in the field. Suicide is one of the leading causes of death in the United States across all age groups. Compared with the general population, individuals with alcohol dependence and persons who use drugs have a 10–14 times greater risk of death by suicide. PowerPoint   PRESENTERS Jana Boocock, MSW is a Senior Prevention Specialist on the Suicide Prevention Resource Center (SPRC) project. Jana’s primary experience includes managing the implementation of suicide and substance use prevention programming in South Dakota communities. Jana received her Master of Social Work in 2021 from Arizona State University and is a Certified Addiction Counselor and Certified Prevention Specialist. In addition to her state and community led prevention experience, Jana also has experience working with youth and adults experiencing behavioral health disorders within the community.   Derrick Newby is a Certified Prevention Consultant who has worked in the field of prevention and public health since 1998. He served as the program coordinator with MidSOUTH Center for Prevention and Training for five years, helping to facilitate the growth of the prevention system. Mr. Newby is a training and technical assistance (T/TA) specialist for the South Southwest Prevention Technology Transfer Center (SSW PTTC). He has also served on the Arkansas Prevention Certification Board since 2020. Mr. Newby earned his Masters of Public Administration from the University of Arkansas, Little Rock in 2004.

Ms. Patton also serves as a research associate with SAMHSA’s Central East Prevention Technology Transfer Center (PTTC), wherein she provides expertise in substance use prevention, mental health, and program management. While serving on this project, she has researched and written numerous PTTC literature reviews on topics such as suicide prevention and opioid use, developed over a dozen different infographics, and co-authored several reports on substance use prevention topics. She has also co-presented on at least 45 webinars on topics ranging from substance use trends to knowledge translation. Emily holds a Master of Science in Abnormal and Clinical Psychology from Swansea University and a Postgraduate Diploma in Criminology and Criminal Justice from the University of Edinburgh.

Students with disabilities are at higher risk of substance misuse. Therefore, it is important that preventionists have the appropriate knowledge and skills to equitably include students with emotional or behavioral problems when delivering prevention services in schools. Much of this work focused on reducing underage drinking in Payne County. As the Students Working Against Tobacco (SWAT) Coordinator, he was responsible for recruiting, training and sustaining SWAT groups at local schools. These student groups sought to complete anti-tobacco advocacy campaigns such as getting their school to pass 24/7 tobacco-free policies. For the last 11 years, he has coordinated various grants that seek to solve local substance use and abuse problems through the use of the Strategic Prevention Framework model by empowering youth.

Chris Davis is the Vice President of Prevention and Youth Support at Community Partnership of the Ozarks in Springfield, Mo.  Chris began his career as a counselor, but 23 years ago started his journey into substance use and violence prevention. Exploring the Science of Connection Behind Promoting First Relationships   May 25, 2022   Description Promoting First Relationships ® (PFR: Kelly et al., 2008) is a brief 10-week home visiting intervention designed for caregivers with infants or young children. PFR is a program that focuses on the quality of the caregiver-child relationship as a source of protection from toxic stress. Providers who learn to use PFR are trained to use strength-based strategies including positive feedback and reflective observation. Together these strategies help caregivers develop a deeper understanding of their child’s behavior which in turn increases their sensitivity to their child’s social and emotional needs. PFR uses video feedback to facilitate reflective observation and discussion. PFR has been evaluated in five randomized clinical trials: two trials in the child welfare system in Washington State; two trials within American Indian tribes; and one trial in a perinatal mental health setting. In this presentation, we will first describe the underlying conceptual model that leverages positive relationships as a source of resilience and then describe the PFR model of intervention. We will also show some videotaped examples of PFR in practice and conclude with a brief overview of the evidence base.    Objectives By the end of the webinar participants will be able to: Identify key elements why focusing on children birth-five promotes overall well-being across the life course. Articulate two strategies that Promoting First Relationships® uses to elevate resilience and caregiver’s strengths to support the Caregiver-Child relationship. Describe PFR home-visiting program and have a basic understanding of relationship-based models of promotion and prevention.   Presenter Monica Oxford, MSW, PhD, is a Research Professor in Child, Family, and Population Health Nursing at the University of Washington and the Executive Director of the Barnard Center for Infant Mental Health and Development. Her research focuses on early parenting and child developmental outcomes for families living in challenging environments. Dr. Oxford’s interest is in how context, parenting, and child characteristics combine to inform particular patterns of child outcomes and how intervention services promote both caregiver and child well-being. Dr. Oxford is also involved in training social service providers throughout Washington on infant mental health, strengths-based practice, and how providers can support caregiver-child dyadic interaction from an attachment-based perspective. Dr. Oxford is the principal investigator of four NIH grants; the first three are randomized control trials aimed at examining the impact of Promoting First Relationships® (PFR: Kelly et al, 2008). Yet, as prevention practitioners, we know that getting caregivers to the table has a direct impact on the success of our efforts. Learn of strategies to prevent youth problem gaming and gambling behaviors that have been adapted from alcohol and drug misuse prevention. PRESENTERS Heather Eshleman, M.P.H., is the Prevention Manager at the Maryland Center of Excellence on Problem Gambling.  She works collaboratively with community organizations, schools, and government agencies to prevent underage and problem gambling as well as oversee special population and youth grants.  She currently leads the Center’s efforts in compiling an underage and problem gambling prevention needs assessment to provide baseline data to guide future prevention strategies.  She served as the Alcohol and Substance Abuse Prevention Supervisor at the Anne Arundel County Department of Health from 2006-2019, overseeing the Substance Abuse Prevention coalitions, the Opioid Misuse Prevention Program, the Strengthening Families Program, the Fatal Overdose Review Team, and Coordination of the Students Against Destructive Decisions Chapters.  Before becoming Supervisor, from 2002-2006, Heather was a Grants Administrator at Anne Arundel County Department of Health.  Heather received her Masters in Public Health from the University at Albany School of Public Health and her Bachelor’s degree in School and Community Health Education from Towson University.  Heather served in the United States Peace Corps in Morocco, North Africa, as a Maternal and Child Health volunteer, working on hygiene promotion and pre-natal care for two years.

  Conducting SBIRT Virtually As with providing any psychological service through virtual methods, specific considerations must be taken when conducting SBIRT virtually. First and foremost, a provider should always confirm the location and contact information of the youth they are working with in order to reconnect, if disconnected, and summon emergency services if necessary. Additionally, when considering conducting SBIRT virtually, there are specific considerations for each component of SBIRT that should be taken into account. For screening, the first consideration that must be taken is how to ensure that your screening procedures are HIPAA compliant. For brief intervention, it can be helpful to provide the youth with additional resources to bolster and support their commitment to reduce substance use. One of the most easily accessed resources is psychoeducational apps, which youth can easily download onto their phones. For referral to treatment, the list of referral sources should include providers who conduct telemental health services. Finally, facilitating a warm handoff, virtually, is still an important step.   SBIRT Implementation Implementing SBIRT in any setting begins with careful planning and discussion among administration and staff. Part of this planning process includes assessing the organization's readiness to change. This can include discussing the settings in which SBIRT will be implemented and the staff will be conducting it. It's also important to discuss how initial staff training and ongoing supervision for SBIRT will be provided. To assist in the SBIRT planning process, administration and staff can discuss the following questions. What are your plans for implementing SBIRT? What screening measures will be used?   Referral to Treatment Referral to treatment is the process of connecting at-risk clients with other service providers for further assessment, medical and behavioral health intervention, or specialized treatment. Providers may sometimes find this step of SBIRT to be difficult if they are not familiar with resources in the community. The good news is that most substance use and prevention professionals have expertise in identifying and locating community resources for clients, and if they don't, then we encourage professionals to consult with their colleagues about resources available in the community. When making referrals, it is important to involve the client and their parent/guardian in the decision-making process. Further, we recommend that providers help their clients schedule a referral appointment while they're still in the office. If available, you can also arrange for an inter-agency facilitator to help with the referral.   This webinar is the fourth in a series of webinars that will highlight the overdose prevention efforts in Region 5. Today will highlight the key challenges associated with opioid misuse/OUD or unintentional fatal mixed stimulant/opioid overdoses within the Black/African American population. Presenters will provide community-informed practices for addressing opioid misuse, overdose and the prevention of OUD in Black/ African American Populations.   LEARNING OBJECTIVES   Provide recent data on prevalence of opioid overdose death rates in the Black/African American population in the U.S. Discuss contextual factors that impact the opioid epidemic and increase overdose deaths in these communities, including challenges to accessing early intervention and treatment. Understand the difference in the ways that Blacks/African Americans use opioids and other drugs compared to White and Latinx populations. Before joining the League, she co-founded and led the Illinois Consortium on Drug Policy at Roosevelt University from 2005-2016 and has worked to pass harm reduction policies in a number of Midwestern states.     Kareem Butler  Director of Learning and Evaluation for the Research and Policy Center, at the Chicago Urban League Kareem Butler is the Director of Learning and Evaluation for the Research and Policy Center, at the Chicago Urban League. In this role, Kareem works enterprise wide to evaluate the performance and resource needs of programmatic departments through a client focused lens to help shape and support programming that is responsive to longstanding community needs and emerging trends.   Preventing Adolescent Depression: Introduction to the Blues Program in the Context of COVID-19   Webinar Date: April 29, 2021   Webinar Description Adolescent depression has been of great concern to prevention practitioners especially given the challenges presented by the pandemic and the stress it has put on young people. This webinar will introduce the research base for adolescent depression prevention interventions. Dr. Paul Rohde will provide an overview of one intervention, the Blues Program, which applies a cognitive-behavioral approach to preventing adolescent depression. The webinar will also highlight how this program can be applied to individual settings.   Objectives By the end of the webinar, participants will: Gain an understanding of the research base for adolescent depression prevention interventions. Obtain a solid introduction to a cognitive-behavioral indicated prevention depression group intervention (Blues Program). Consider how they can apply this intervention or components of the program to their specific settings.   Presenter Dr. Paul Rohde is a Senior Research Scientist at Oregon Research Institute (ORI) and has over 30 years of experience as a research scientist with a focus on the etiology, treatment, and prevention of adolescent and young adult depression, eating disorders, and obesity. He has managed 8 federally funded research projects and been the Co-Investigator on an additional 20 federally funded studies. Most of these studies have been randomized controlled trials (RCTs) evaluating prevention or treatment interventions in young people. He has published over 165 peer-reviewed articles, chapters, and books and has served on several journal and federal grant review boards. She offers over nine years of expertise in data analysis, program management, and evaluation. Under the District of Columbia Department of Behavioral Health (DBH), she provides data analysis, programmatic support, and training and technical assistance (TTA) for multiple contracts that serve youth and families in the district. She also serves as a research associate with the Prevention Technology Transfer Center Network (PTTC) Central-East region. In this role she provides expertise in substance use prevention, mental health, and program management. Prior to joining Carnevale Associates, she worked for CSR Inc., serving as the lead analyst for the Edward Byrne Memorial Justice Assistance Grant (JAG) Program, which is the largest source of federal justice funding in the country under the direction of the Bureau of Justice Assistance (BJA). Emily holds a Master of Science with Merit in Abnormal and Clinical Psychology from Swansea University and a Postgraduate Diploma in Criminology and Criminal Justice from the University of Edinburgh.    Josh Esrick, MPP is a Senior Policy Analyst with Carnevale Associates, LLC. Mr. Esrick has extensive experience researching, writing, evaluating, and presenting on substance use prevention and other behavioral health topics. He has developed numerous Substance Abuse and Mental Health Services Administration (SAMHSA) Prevention Technology Transfer Center and Center for the Application of Prevention Technologies’ products and trainings. Among other projects for SAMHSA, he has helped write annual Data Collection, Analysis, and Reporting reports, conduct evaluations of the Strategic Prevention Framework State Incentive Grants, and wrote the monthly Financing Focus for the Center for Financing Reform and Innovation. Mr. Esrick has also published in the International Journal of Drug Policy on considerations surrounding marijuana legalization and in Drugs: Education, Prevention and Policy on the modern usage of fear-based prevention messaging. He holds a Bachelor of Arts in Political Science from the State University of New York at Buffalo and a Masters of Public Policy from George Washington University.

COURSE DESCRIPTION Session 1: Understanding the Science of Trauma, Illuminating Prevention Action provides direct instruction on the most powerful determinant of health, toxic stress, and explores prevention actions that have the power to shift the trajectory of well-being for future generations. With a presentation on a cluster of emerging scientific findings that include resilience research like the Adverse Childhood Experiences (ACE) study, developmental neuroscience, and epigenetics, participants review the ways that toxic stress during development can affect cognition, relationships, health, and patterns of crisis and coping that affect risks for substance use and other challenging life experiences. Through that link, communities accelerate the intergenerational changes necessary to improve health and quality of life.
